Program Overview

Overview

North Dakota has a Medicaid HCBS system with four children's 1915(c) waivers, all of which count only the child's income. There is no Katie Beckett/TEFRA route, but a Medicaid buy-in for children with disabilities also exists.

Katie Beckett or similar pathway

North Dakota does not have a Katie Beckett or TEFRA pathway. It instead runs four 1915(c) children's waivers — the Medically Fragile Children's Waiver (ages 3-21), Children's Hospice Waiver (0-21), Autism Spectrum Disorder Waiver (0-17), and Traditional ID/DD Waiver — all of which count only the child's income. A buy-in program, Children with Disabilities Medicaid Coverage, also exists.

Child-relevant waivers

Children's routes include the Medically Fragile Children's Waiver, Children's Hospice Waiver, Autism Spectrum Disorder (0-17) Waiver, and Traditional ID/DD Waiver, plus the Children with Disabilities Medicaid Coverage buy-in.

Disability documentation, level-of-care assessment, and financial information are typically required.

Waitlist information

The Medically Fragile Children's and Children's Hospice waivers have no waiting list. The ASD waiver has a waitlist of about 140 children.

Services & Support

Services available

DD Waiver can cover residential services, day services, employment supports, and in-home supports.

Parent caregiver pay

North Dakota's Family Paid Caregiver Pilot Program (since April 2024) pays caregivers of children enrolled in the ASD, Medically Fragile, Children's Hospice, or Traditional ID/DD waivers.

Equipment exchange & assistive technology

IPAT / North Dakota Assistive Technology (ipat.org) — free device demonstrations, short-term loans, reuse programs, and AT information statewide.

Mental health and Medicaid coverage

North Dakota Medicaid covers behavioral health services including outpatient therapy, psychiatric evaluations, medication management, and crisis services. The Division of Behavioral Health (DBH) within HHS oversees public mental health. Community mental health centers provide most direct services. North Dakota has faced rural mental health workforce shortages and uses telehealth to extend access. Children can access mental health services through the EPSDT benefit. The state has also invested in school-based mental health supports.

Additional notes

North Dakota's smaller population means some programs have limited capacity. Families should contact DHS directly to ask about current availability and any expected wait times.
